Before you apply for a job with HAAS, please review the information in the PCA Job Description. This will help you to decide if you want to be a Personal Care Attendant. Then fill out and submit the following forms:
We will need a copy of your Drivers License or your State ID card along with another form of identification in order to complete the background check (Social Security Card, Passport, Birth Certificate, etc.) and to complete your I-9 paperwork.
In addition, the Minnesota DHS requires several other forms to enroll you as a PCA and affiliate you with our agency. You will need to fill out these forms and sign them (electronic signatures will not be accepted). If you wish to print them out and have them ready to turn in to us, you may. Otherwise, we will have copies at the office that you may fill out there. You can not send these forms to us electronically as DHS will not accept any forms that do not have a signature and/or initials signed in ink.
If you do not already have your PCA certificate from the state of Minnesota, you will need to complete the online training and test. See instructions here.
To be employed we will need you to complete W-4 tax information and the I-9 Employment Verification. You will need to provide copies of IDs as listed on page 3 of the I-9 form. You may email the forms and copies of your identification documents to us at haas@haascares.com or drop them off in person but we will need to keep a copy of those on file with your completed I-9.
DHS does not allow PCAs to work more than 310 hours per month. As PCAs may work for more than one agency, HAAS can not know if a PCA is running over that amount of hours. We ask all our PCAs so sign an agreement that they will not work for more than 310 hours per month with the understanding that we will ask the PCA to pay back the overage hours if they violate this agreement.
HAAS recommends using Direct Deposit for your paycheck. You may always chose to get a paper check that can be picked up at the office (5851 Duluth st, Suite #302, Golden Valley, MN 55442) on payday. The form asks for a voided check to be attached. That requirement insures that we get the information for your payroll deposit correct. You may take a picture of a check and send it to us if you choose to submit this form electronically. You may also print out the hardcopy form, fill it out and drop it off with a voided check at the HAAS office.
