— What you’ll do

The day-to-day.

  • Conduct assessments and contribute to care plan development.
  • Provide skilled-nursing tasks as authorized in client care plans.
  • Supervise and train PCAs and 245D direct support staff on health-related tasks.
  • Coordinate with primary-care providers, case managers, and families.
  • Document visits and clinical observations.
  • Respond to changes in client condition and adjust care plans accordingly.
— What we’re looking for

Qualifications.

  • Active RN or LPN license in good standing in Minnesota.
  • Prior nursing experience preferred — home health, geriatrics, disability services, or community health especially.
  • Willingness to travel to client homes across the Twin Cities metro.
  • Comfort with electronic documentation and care planning systems.
  • Excellent communication and supervisory skills.
