— What you’ll do

The day-to-day.

  • Supervise caregivers across multiple client homes.
  • Conduct intake assessments and develop care plans alongside clients and families.
  • Coordinate with case managers, county workers, and waiver coordinators.
  • Hire, onboard, and continuously develop direct-support staff.
  • Ensure compliance with Minnesota 245D and CFSS standards.
  • Document supervision visits and audit care plan execution.
— What we’re looking for

Qualifications.

  • Bachelor's degree in social work, psychology, nursing, special education, or a related human-services field — OR equivalent qualifying experience per Minnesota QP paths.
  • At least one year of supervisory experience in home care, community supports, or a related setting.
  • Familiarity with Minnesota waiver programs (CADI, BI, CAC, DD, EW, AC).
  • Strong written and verbal communication skills.
  • Bilingual / multilingual experience strongly preferred.
  • Comfort with documentation, audits, and compliance work.
